Consent and Acknowledgement

I, the undersigned, am the legal parent or guardian of the above-named child and give my full consent for my child to participate in training sessions at [Gym Name]. I acknowledge that participation in physical training involves inherent risks, including injury, which may result from accidents, negligence, or unforeseen circumstances. I agree to the following:

Assumption of Risk:

I understand that my child's participation in these training sessions is voluntary and that there are inherent risks involved. I accept full responsibility for any injury or harm that may result from participation.

Release of Liability:

I hereby release, waive, and discharge Unified Strength, its owners, employees, trainers, and agents from any and all claims, demands, or causes of action arising out of or in connection with my child's participation in training sessions, including claims for negligence, breach of contract, or any other cause.

Medical Authorization:

In the event of a medical emergency, I authorize Unified Strength Gym and its representatives to secure any necessary medical treatment for my child. I agree to be responsible for all costs incurred as a result of such treatment.

Compliance with Gym Rules:

I confirm that my child will follow all instructions and safety protocols set forth by Unified Strength Gym and its trainers.

Parental Vouching for Gym Skills and Independent Training:

I affirm that I have assessed my child's ability to safely navigate and use gym equipment. I am confident in my child's skills and judgment to train independently at Unified Strength Gym. If, at any time, I feel uncertain about my child's capability to train safely on their own, I will proactively schedule an introductory or refresher session with a trainer to ensure their readiness and safety.

Gym Staffing

I am aware that Unified Strength Gym is not always manned and that a member of staff may not always be present.
